Am I covered if I travel outside the United States?
Expenses for care or treatment received outside of the United States or its territories, except for unexpected emergency situations while traveling, are excluded. For emergent care in other countries, you will need to pay your bill and submit it to the Claim Administrator for reimbursement pursuant to applicable Plan provisions. We recommend you pay with a credit card as it automatically adjusts the amount to U.S. dollars.
How do I add a dependent to my health care benefit plan?
To add a dependent to your health care benefit plan, please visit your local Human Resources/Benefits Department. You are required to bring a birth certificate at the time you wish to enroll your dependents.
How long does it take to receive a new ID card?
Typically, it takes up to two weeks to receive a new ID card. In the meantime, you may print a temporary ID card by logging onto

How much will I pay at the pharmacy?
Prescription drug claims are processed through the Plan the same way as medical claims. There are no additional copays necessary to receive prescription drugs. Generics are covered 100% after the Health Reimbursement Account (HRA) has been exhausted. Brand name drugs without a generic equivalent are processed at 90% once the Deductible has been met. Brand name drugs with a generic equivalent are processed at 90% once the Deductible has been met. Please note that for these types of medications, an ancillary charge will be applied in addition to the cost of the medication.
What is a Deductible?
The Deductible includes your Health Reimbursement Account (HRA) and your Remaining Deductible. The amount of your Deductible is based on your level of coverage and the effective date of your coverage.
Does LSU First have a prescription drug formulary?
LSU First does not have a formulary for prescription drugs. All FDA medications are covered by the Plan.
What is an Explanation of Benefits (EOB)?
An EOB details how a specific claim was processed, any Member responsibility due, and the reasons for denying payment, if applicable. EOBs are available online only, unless you request for EOBs to be mailed to your home. You may do this by logging onto

Why do generic drugs cost less?
Generic drugs tend to cost less because the companies that make them do not have to recover the cost of research and development.
Why isn't my dependent's name listed on my ID card?
As a subscriber, only your name will be listed on any ID card. However, any dependent listed in your health care benefits plan will be covered according to your policy.
What is a First Choice Provider and how are claims paid for First Choice Providers?
A Healthcare Provider that has met certain established standards and who has agreed to accept certain fixed fee payments from the Plan for Covered Expenses. Claims are paid at 100% after your Health Reimbursement Account (HRA) has been exhausted. Your HRA Rollover will not be used to pay for First Choice Providers.
I visited an In-Network facility but received a bill from an Out-of-Network Provider. Why is this?
An Out-of-Network Provider is a health care provider that has not entered into a contract or agreement directly with a network of providers accessed by LSU First. Providers cannot be required to become Contracted Health Care Providers. The fact that a hospital or other facility is an In-Network facility does NOT mean that all of the Providers furnishing services at that facility are In-Network Providers. Facility-based physicians or Providers may not be Contracted Health Care Providers and you may be responsible for charges in excess of Plan payments. Currently, Louisiana law allows Providers to chose whether they want to join the facility network. Typically these Providers are known as hospital based physicians (example: anesthesiologists, radiologists, and emergency room physicians).
Is LSU First a true Medicare Supplement Plan?
No. LSU First will coordinate benefits after Medicare by estimating what LSU First would have paid if primary. LSU First will then subtract any amount paid by Medicare. If this amount is greater than what LSU First would have paid, LSU First will process the remaining portion through the plan as any medical claim. If this amount is less than What LSU First would have paid, no additional amount is due and the bill is satisfied.
